The shaft that comes out of the case that the shift lever broke off at the case how hard is it to replace?

I'm refitting my engine back to frame and I am wondering what is correct procedure to srew back this bolt (see picture below)? Do I need to srew only middle bolt, or for why is there outside as well? Is there any special tool for it?

I just got me a smoking deal on a pair of Pirelli Super Corsa SPs, but I'm not so sure if the 200/55 rear tire I got would fit on my 2002 Mille R. Has anyone tried it? I currently have Dunlop GPs 190/55 on the rear and there's very little clearance between it and the chain.
